Blooming Connections Adult Day Program

1️⃣ What is an Adult Day Program and what services do you provide?

Our program offers structured day services for adults with intellectual and developmental disabilities, focusing on social engagement, life skills, community integration, recreation, and personal growth. We provide specialized habilitation, skill-building activities, sensory exploration, arts & crafts, music, community outings, and communication support.

2️⃣ Who is eligible to attend Blooming Connections?

We support adults (18+) with Intellectual and/or Developmental Disabilities (IDD) who have Medicaid waivers (DD, SLS, or CES), or private pay. Participants should benefit from a supportive, structured day environment and have goals related to socialization, independence, and community integration.

3️⃣ What insurance do you accept?

We accept:

  • Medicaid.

  • Self pay

4️⃣ What type of activities do participants do each day?

Activities are individualized and purposeful, including:
⭐ Life skills (cooking, money handling, hygiene)
🎨 Creative arts & crafts
🧘 Sensory and relaxation rooms
🎵 Music, movement, and games
🤝 Social skill building & communication support
🌳 Community outings and volunteering

5️⃣ Do you offer transportation?

Yes — we provide Non-Medical Transportation (NMT) for eligible participants. Transportation can be arranged from home to the center and back, depending on location and availability.

6️⃣ Can you support non-verbal or high-support individuals?

Absolutely! We support individuals at all support levels, including those who are non-verbal, use mobility devices, require 1:1 care, or have sensory or behavioral needs. Our staff receive training in communication devices, behavior support, personal care, and person-centered approaches.

7️⃣ What are your program hours?

Our standard hours are:
Monday to Friday — 8:00 AM to 4:00 PM
We offer full day, half-day, and flexible scheduling to match each individual’s service plan.

8️⃣ Can family members or caregivers visit the center?

Yes! We encourage family involvement. Caregivers can schedule a tour, participate in planning meetings, or join us for special events and celebrations.

9️⃣ Do you provide meals and snacks?

We provide light snacks (including our Coffee & Snack Bar), but families typically send lunch based on dietary needs. We can assist with mealtime support, adaptive utensils, and special diets if needed.
